Terms, Conditions, Accessibility

By submitting any form of payment to Midwest ADP, or by scheduling services with Midwest ADP, I understand and accept:

  • To receive communications from Midwest ADP via e-mail, text message, fax, phone and/or postal mail, and that these communications may include confidential information about my classes, SATOP programs or court case/probation information.  I understand that I may rescind permission to receive certain types of communications from Midwest ADP in writing at any time.  I further understand that once Midwest ADP sends communications to me in any format I am assumed to have received, read and agreed to those communications until I otherwise notify Midwest ADP.
  • I further understand that any fees paid are refundable only prior to services being provided. If I schedule a class and do not attend without at least 48 hours written or verbal notice, I understand I may not be eligible for a refund, and may have to pay all or part of the class fee in order to schedule for a new date.
  • If I attend class and am unsuccessfully discharged or otherwise fail to complete the class for any reason, I understand that I’m eligible only for a refund equal to the difference between amount paid and the value of services provided (SATOP CIP/SROP only). If I attend any other class and am unsuccessfully discharged during class, I understand I may not be eligible for a refund, and may have to pay all or part of the class fee in order to schedule for a new date.
  • Midwest ADP is certified by the Missouri Department of Mental Health to provide Intensive Outpatient Treatment and Outpatient Treatment and the following SATOP programs: Offender Management Unit (OMU or SATOP Screenings), SATOP Level 1 Offender Education Program (OEP), SATOP Level 2 Weekend Intervention Program (WIP), SATOP Level 3 Clinical Intervention Program (CIP) and SATOP Level 4 Serious and Repeat Offender Program (SROP).  Midwest ADP clinical staff are registered, certified or licensed by the Missouri Credentialing Board.  Midwest ADP makes no claim that it or its personnel are certified to provide services in any other state besides Missouri, and for the purposes of your SATOP program, you agree that services are provided in Independence, Missouri, though your actual location may be different.  By participating in our SATOP services you expressly acknowledge that Midwest ADP is adhering to the Missouri Department of Mental Health and SATOP rules and certification standards only, and Midwest ADP makes no claim or warranty that our SATOP services meet any other state’s rules or certification standards.
