When you’re ready to complete your questionnaire, click here to begin. You’ll need your authorization code to start.  Once you finish the questionnaire, select a time and date for our counselor to call you. IF YOU ARE UNDER THE AGE OF 18, OR WOULD PREFER TO TAKE THE TEST IN SPANISH PLEASE DO NOT TAKE THE TEST – PLEASE CALL OUR OFFICE AT 816-836-2220 TO ARRANGE TO TAKE THE TEST.

Our counselor will call you during your selected time window (within 60 minutes of your selected time) for a quick interview.  They’ll review your questionnaire answers, your driving record, and ask you some questions about the time you were pulled over.  Finally, they’ll make a program assignment based on these factors.
